• Child Care Food Program Enrollment Form
• Physical
• Child habits/care survey
• Infant/toddler care sheet if under age 2
• Medication form if needed

• *Alternate schedule /Exemption card for immunizations.  These are obtained from doctor or the Health Department in accordance with Missouri law. 
• *Parents have a right to request at enrollment or following enrollment information regarding if there are any exemptions for immunizations in accordance with Missouri law.  

Items to bring:

• Change of clothes- 2 Full sets 
• Outerwear for the weather- cold- hat/gloves/coat/boots
• Swimsuit for water play in summer for hot days
• Tooth brush
• Blanket for rest time (and comfort item if desired)
• Pullups/Diapers/ Wipes if needed/Diaper cream
• Formula/Breastmilk bottles if needed
• Any special items your child requires
• Car seat left on any outing days

Optional:

• Holiday or birthday treats to share if desired and by prior arrangement
• Special books to share in story time

Items not to bring: 

• Special toys that cannot be shared or are valuable
• Personal snacks for just your child
• Clothes that are very nice and are not good for playing
• Clothes that are difficult for children to take on or off for restroom independently/ or make  changing difficult
